Supporting patients’ social determinants of health (SDOH) and health-related social needs (HRSNs) is among the most impactful activities for pharmacy teams. Studies show up to 50% of outcomes are driven by social, economic, and cultural factors while clinical care accounts for only 20%.

CPS leaders Lily Duong, PharmD, SVP of Clinical Development, and Shanece Green, PharmD, RPh, MSPH, MBA, Director of Health Equity will present “The Digital Bridge: Pharmacist Innovating Targeting Social Health Gaps.”
