Background

  • It is recognized that enhanced management during the transition to home can lead to improved patient outcomes and further decrease readmissions.
  • Pharmacist-led medication reconciliation occurring at or post- discharge can resolve discrepancies prior to the outpatient transition.
  • Studies have shown medication reconciliation alone does not show a significant reduction in 30-day readmission rates.
  • Literature details a proposed framework for implementing of transitions of care (TOC) interventions and process measures to track success for further optimizing the quality of pediatric care.
  • Models show promise in pediatric TOC but have not developed financially sustainable workflows and revenue integrity structure.6Health system specialty pharmacy (HSSP) embeds pharmacists directly into clinics to ensure patients with chronic and complex diseases receive personalized, high-touch care.

Objectives

To explore the expansion from the current discharge counseling service offered by a pharmacist, who is embedded in a pediatric neurology service, into a billable TOC management service.
