CPS HSSPs to present “Health System Specialty Pharmacy: Ensuring Continuity of Care During Provider Transitions” at AMCP 2024 Annual Meeting.
April 2024
A new clinical outcomes study from CPS' health system specialty pharmacy (HSSP) network reveals that HSSPs are adept at filling care gaps during transitional periods.
The CPS team will present the findings from the "Health System Specialty Pharmacy: Ensuring Continuity of Care During Provider Transitions" study at the Academy of Managed Care Pharmacy (AMCP)'s 2024 Annual Meeting, April 15-18, 2024, in New Orleans.
The study authors included Carly Giavatto, PharmD; Brett Shaffer, PharmD; Audrey Rush, AA, CPhT; Amy Evans, PharmD, CSP; Jessica Mourani, PharmD; Casey Fitzpatrick, PharmD, BCPS; and Ana Lopez-Medina, PharmD. They found:
- Disruptions during care transitions can impact patients' therapy adherence and clinical outcomes.
- HSSP teams play a pivotal role in maintaining continuity of care, particularly during provider transitions.
- Embedding HSSP teams to support care transitions can help hospitals and health systems improve patient satisfaction.
Dr. Giavatto recently emphasized the impact of the study's findings: "Transitional periods can lead to therapy gaps like medication non-compliance and disease progression. HSSPs, with their unique services, can be the solution. The findings underscore the importance of HSSP involvement in specialty patient care."
