How CPS pharmacy teams prevent medication errors, strengthen care continuity

In 2023, the leaders of a large health system in the Southeast U.S. identified that their Transitions of Care (TOC) program needed more support. As a network with multiple hospitals and external Emergency Departments that provide care for a community of more than 40,000 patients, they decided to innovate—with the help of the experts at CPS Solutions, LLC (CPS).

Care transitions, particularly hospital admission and discharge, are critical times for patients. People often need close support as they learn new care plans and adjust medication and therapy regimens.

In just nine months, the CPS TELEpharmacy® team worked with this health system to transform TOC into a pharmacy-led program, saving $3.074 million between March 1, 2023 – November 30, 2023 while also improving patient care and quality.
