Members of our team had the opportunity to present a new poster abstract at the ASHP 2023 Midyear Clinical Meeting held December 3-7.
Background
Evidence from leading national agencies indicates health care disparities continue to exist across diverse populations despite advancements in medical care and the call to address multi-layered factors influencing health equity (HE). To lead practice-related efforts in advancing health equity, CPS—a healthcare service provider that partners with 800+ facilities nationwide to optimize pharmacy best practices—created the Health Equity and Systems Thinking (HEST) pharmacy fellowship program. This training pathway combines objectives- and project-based learning experiences emphasizing systems thinking approaches to provide our fellow a deeper understanding of the social determinants of health (SDOH), health-related social needs (HRSN), and key issues affecting medically underserved populations, thereby empowering a more culturally competent pharmacy workforce.Objectives
- Create a specialized postgraduate program in HE and systems thinking and develop associated resources, trainings, and assessments
- Improve organizational health literacy and patient outcomes through a more culturally responsive pharmacist workforce who integrates universal precautions
- Implement SDOH/HRSN screening, trauma-informed care, and systems thinking into the medication use encounters
- Promote advocacy and partnerships to improve access and elevate clinical cultural competence
