How a Transitions of Care Pilot Study Increased HCAHPS Scores and Reduced Readmissions

Joint Township District Memorial Hospital, located in St. Marys, Ohio, is a
nonprofit, 123-bed general acute care hospital.

 

Part of the Grand Lake Health System, the hospital provides emergency care for injuries, as well as sudden and severe illnesses. Committed to serving the local community with compassionate care, advanced technology, and exceptional expertise, Grand Lake Health System offers a comprehensive range of primary, acute, and therapeutic health services to residents of the Grand Lake region in western Ohio. They also offer surgical, emergency, transitional, and behavioral care services, as well as a wide-ranging ambulatory offering.

 

The Situation


Hospital leadership collaborated with CPS TELEpharmacy®, a division of CPS®, to conduct a pilot study to measure the impact of pharmacist-driven Transitions of Care (TOC) services at their site.

The pilot program assessed the following TOC service areas:

  • Admission Medication Reconciliation
  • Discharge Medication Reconciliation
  • Discharge Counseling
  • Post-discharge Follow Up

 

By prioritizing value delivery for both Joint Township Division Memorial Hospital and its patients, we were able to show the measurable benefits that its Transitions of Care program had delivered.
