Members of our team had the opportunity to present a new poster abstract at the ASHP 2023 Midyear Clinical Meeting held December 3-7.

CALM in HSSP

Background

  • Migraines are classified as a debilitating neurological condition in which a patient experiences recurrent headache attacks.
  • In terms of years lived with disability, migraine is the second most debilitating disease, affecting about 1 in 7 individuals worldwide.
  • One way to improve care in high-volume complex disease states is through population health management (PHM), which focuses on stratifying patients based on their health risk with the goal of adapting interventions to meet the needs of each subgroup.
  • This model demonstrated positive outcomes in health system specialty pharmacy (HSSP) managed disease states, such as multiple sclerosis, atrial fibrillation, hepatitis C, and diabetes. However, the risk stratification protocol in the management of migraine is limited.

Objectives

Determine the impact of a HSSP population health management protocol in patients with migraine.
