Outcomes Study: Technology-Driven Social Determinants of Health Screening for Patients with Neurological Conditions

Members of our team had the opportunity to present a new poster abstract, "Technology-Driven Social Determinants of Health Screening for Patients with Neurological Conditions", at the NASP 2025 Annual Meeting & Expo.

Background:

• Social determinants of health (SDOH) are the conditions in which individuals are born, grow, work, live, and age, contributing to approximately 50% of health outcomes.^1-2

• Health-related social needs (HRSNs) represent individual-level barriers that directly impact a patient's ability to access and engage in healthcare. By leveraging technology to integrate screening tools into pharmacy workflows, pharmacists can systematically assess patients' HRSN and improve care coordination.^1-2

Objectives:

• To streamline HRSN identification, SDOH data visualization using technology-driven approach, and referral process to social work and patient assistance resources within a health-system specialty pharmacy.
