Background:
- Patients with severe mental illness are at significantly increased risk for developing type II diabetes mellitus and metabolic syndrome.
- Inadequate management of type II diabetes and metabolic syndrome can result in costly and life-altering macrovascular complications.
- Continuous glucose monitoring (CGM) has been shown to decrease hypoglycemia rates and increase the time spent with the target glycemic range compared to traditional blood glucose monitoring.ยน
- Due to an established diabetes management program, pharmacists at an inpatient behavioral health facility are well positioned to create a workflow aimed at transitioning eligible patients to CGM.
Objectives:
- The objective of this study is to describe the development of a workflow used to implement CGM in a pharmacist-led diabetes management program in an inpatient behavioral health hospital.
